At the office of Dr. Jeffrey Schildhorn, we pride ourselves on creating an efficient and productive environment. Our three New York City office locations provide orthopedic consultation, diagnosis, and surgery. Many of our patients live and work in New York- a fast-paced environment- and we understand that our patients’ time is valuable and can be limited. To set expectations and make your visit with us as thorough as possible, please read through the following prior to your first appointment.
Dr. Schildhorn invests time in understanding each new patient’s personal healthcare needs, which often lie outside of orthopedic surgery. He considers each patient's lifestyle prior to creating a healthy plan of action that promotes good medicine. To allow for enough time to be properly assessed, appointments can last between 30 minutes and 1 hour, depending on the reason for your appointment. Please plan your time accordingly.
To cancel or reschedule your appointment, please give us 48 hours notice so we may offer your appointment time to another patient. As our three orthopedic offices reside in New York, we understand that time is a valuable asset, so when we have the ability to see a patient sooner than expected, we make every effort to do so. There will be a no show fee of $100 for patients who cancel with no notice.
It is highly recommended for first time patients to bring or wear shorts/leggings.

The goal of your first appointment with Dr. Jeffrey Schildhorn is to thoroughly assess your health, injury, strength and lifestyle as it pertains to orthopedic healthcare. It is important to collect detailed information in regards to your orthopedic history and health issues prior to your first visit. This will allow you to discuss an all encompassing plan of action with Dr. Jeffrey Schildhorn that fits your needs.
At the time of your first visit, you will need to provide:
- Insurance card
- Form of identification
- Any relative medical reports (see examples below)
- Form of payment **We currently do not accept credit cards for transactions under $100.
- Shorts / Leggings / Sports bra

For your appointment to run as smoothly as possible, please bring any necessary diagnostic imagery and operative reports you currently have on hand. You can send your reports in advance through the mail to:

Helpful reports include:
- Recent MRI Films and typed MRI report
- Recent X-Ray Films and typed X-Ray Report
- Operative reports, photos, or video of prior surgery. (Ask your previous surgeon for these)
- Physician reports pertaining to this specific problem.
Your insurance information is collected prior to arrival at one of our New York offices. However, please bring insurance card(s) with you in the event an insurance authorization for services is required. Our billing staff is happy to help you with understanding any insurance questions and can assist you in obtaining reimbursement from your insurance company.
It is important to note that although the offices of Dr. Jeffrey Schildhorn accepts most insurances, your insurance provider may require you to meet a deductible.

Payment is required at the time of service and payment information will be collected upon arrival to the office of Dr. Jeffrey Schildhorn. Payment information can also be collected at the same time as your insurance information prior to your appointment. The offices of Dr. Jeffrey Schildhorn accept cash and check, however only accepts credit card payments for over $100.
